Prior to accessing the application form, applicants should review the Certification Criteria.
SGP allows a facility a 12 month time frame to meet all criteria (including successful completion of an on-site audit) in order to become SGP Certified.
To begin the application process, download and fill out the application below. Email the completed application to firstname.lastname@example.org or fax it to +1 (703) 359-1333.
Upon fee submission the application will be reviewed by SGP staff for completeness and accuracy. If there are questions, the facility will be contacted for clarification.
Once the application is deemed complete, the facility will be notified and asked to proceed to the next certification steps.
